Membership Application
Regular Members are those who have complied with all the membership requirements and are entitled to all the rights and privileges of membership.
Aside from the general requirements, a regular member must also:
Be a holder of common shares;
Subscribe and pay the minimum share capital requirement for regular member;
Undertake to uphold the By-Laws, policies, guidelines, rules and regulations promulgated by the Board of Directors and the General Assembly;
Continually patronize the products and services of the Cooperative;
Contribute regularly towards capital build-up, regular savings and other mandatory dues prescribed and approved by the General Assembly;
Not have been convicted of any administrative, civil or criminal cases in the past 3 years; and
Be of good moral character
The applicant for regular membership must have complied with the following requirements:
- Attended and completed the prescribed Pre-Membership Education seminar
- Certificate of completion of the PMES
- Fill out the cooperative’s membership application form
- Submitted 1 piece 2x2 ID picture
- Submitted a copy of Birth Certificate

Subscribed and paid the required minimum share capital of P5,000.00 which he/she can pay at once or little by little over time,
membership fee of P200.00, damayan fee of P200.00, annual blood Samaritan fund of P100.00, and P100.00 initial savings deposit.
He/She must also sign a subscription agreement - Submitted a photocopy of at least 1 primary valid ID or 2 secondary valid ID’s
Primary IDs are:
- SSS/GSIS
- Drivers licensed (not expired)
- PRC license (not expired)
- Passport (not expired)
- Tax Identification Number (ID)
Secondary ID’s are:
- HMDF (Pag-ibig ID)
- Philhealth ID
- Senior Citizen ID
- Updated Postal ID (Not expired)
- School ID
- Employee’s ID
Associate members are those who have no right to vote nor be voted upon and are entitled only to limited rights, privileges, and membership duration as provided in the by-laws of the cooperative, the Philippine Cooperative Code of 2008, and its Implementing Rules and Regulation.
An associate member who meets the minimum requirements of regular membership and continues to patronize the cooperative for two (2) years, and signifies his/her intention to remain a member shall be considered a regular member.
An individual who applied for membership but has not yet complied with the minimum requirements of the cooperative shall also be considered as an associate member.
Aside from the general requirements, an associate member must also:
- Subscribe to at least one shares preferred share/s for a total of P100.00
- Undertake to uphold the By-Laws, policies, guidelines, rules and regulations promulgated by the Board of Directors and the General Assembly
- Continually patronize the products and services of the Cooperative
- Contribute regularly towards capital build-up, regular savings and other mandatory dues prescribed and approved by the General Assembly
- Not have been convicted of any administrative, civil or criminal cases in the past 3 years
- Be of good moral character

- Attended and completed the prescribed Pre-Membership Education seminar

- Paid the membership fee of P200.00, damayan fee of P200.00, annual blood Samaritan fund of P100.00, P100.00 initial share capital, and P100.00 initial savings deposit. He/She must also sign a subscription agreement
- Submitted a photocopy of at least 1 primary valid ID or 2 secondary valid ID’s
